Survey

 At the end of your activity we ask that you and your child fill out the surveys available. All information is completely voluntary. We may use some or all of your responses/testimonials on our website and/or reports to donors and contributors. We will make reasonable efforts for you and your personally identifying information and responses to remain anonymous. If you would like to send us a picture/image of your child participating in their activity please feel free to do so. We do ask that the photo does not contain any identifying aspects of individuals other than YOUR child. Furthermore, we need permission from you, the parent/guardian, to use the photo submitted and the manner we may use the image, for example on our website, reporting purposes and solicitation of donations. Completing the surveys and photo submission will go a long way in helping us secure more funding to help more families. Thank you for your help!
